North Central C-MED Tape Request Policy North Central C-MED will provide copies of tape recorded
information, free of charge to Sponsor Hospitals, Receiving Hospitals, and
Responding Agencies involved in the incident in question, as well as Municipal
Officials of the community where an incident occurs. To facilitate in the retrieval of the taped incident, it is imperative that the following guidelines be followed: 1. Request must be made in writing on the letterhead of the agency you are representing.2. Agency must in someway be directly involved or linked to the incident in question.3. Provide complete and accurate information to as many of the following questions as possible: C-MED Case Number; Date of Incident; Time of Incident; Town of Incident; Nature of Incident; Ambulances Responding to Incident; Medical Control Hospital; Receiving Hospital.4. Mail of fax your request to North Central CT EMS Council. 5. Allow 5 business days from our receipt for processing of a tape request. |
